What intake will ask

A useful referral starts with the right questions.

  • Graft and donor-site location, procedure date, dressings, and activity restrictions
  • Color, drainage, odor, pain, swelling, bleeding, and recent changes
  • Pressure avoidance, immobilization, elevation, and caregiver or facility support
  • Upcoming surgeon follow-up and the secure pathway for images or records when requested

Whole-patient context

The wound is only one part of the plan.

Primary-care continuity, diabetes, circulation, edema, mobility, nutrition, medications, pressure exposure, caregiver capacity, current setting, and recent care transitions may all affect the next step.

Primary care context

Identify the active PCP relationship, chronic conditions, medications, and recent medical changes.

Setting and capacity

Clarify whether the patient is at home, in a facility, receiving home health, or transitioning after discharge.

Communication pathway

Identify the clinician, facility, caregiver, or referral source who should receive the next update.

Start with the right path

Request a review of the care need.

Provide the minimum intake details, location, setting, wound concern, and best contact. The team will confirm whether additional information is needed through a secure channel.
