A simple checklist for facility wound referrals
For DONs and floor staff: a little prep makes a wound referral go fast and smoothly. Here’s what to have ready.
Have these ready
The resident’s name and room; the wound’s location, type, and how long it’s been present; recent treatments and dressings; relevant history like diabetes or vascular issues; and the urgency.
Flag urgency clearly
Note if there are signs of infection — spreading redness, drainage, odor, fever — or a wound that’s rapidly worsening, so it gets prioritized appropriately.
Name a point of contact
Tell us who on your team to coordinate with and the best way to reach them, so updates and the plan get to the right person.
Then hand it off
Call or email with the above and we take it from there — on-site assessment, treatment, a clear plan, and clean documentation for your records.
